We begin by understanding the participant’s goals, preferences, routines, communication and risks. Supports are then agreed in writing, delivered by suitably matched workers and reviewed as needs change.

- Identifying preferences, strengths and support needs
- Liaising with participants, nominees and relevant providers
Availability and funding eligibility depend on individual circumstances, location, service capacity and the participant’s current plan or private arrangement.
