We listen to the older person and their representatives, understand established routines and confirm what has been authorised before service begins. Support is provided respectfully, with clear communication and documentation.

Support may include

• Transport to medical and allied-health appointments
• Support before, during and after agreed appointments
• Grocery shopping and essential household errands
• Pharmacy visits and collection of approved items
• Banking, post-office and other local errands
• Transport to community activities and social visits
• Planning accessible routes and documenting agreed travel

The exact service, frequency and tasks depend on the person’s assessed needs, approved Support at Home care plan, available budget, Trilogy Care arrangement, location and CCTS service capacity.
