Reliable support, agreed around the person.
We listen to the older person and their representatives, understand established routines and confirm what has been authorised before service begins. Support is provided respectfully, with clear communication and documentation.
Support may include
- Showering, bathing and personal hygiene assistance
- Dressing, grooming and presentation
- Toileting and continence-related assistance within the agreed care plan
- Mobility, transfers and safe movement around the home
- Morning and evening routines
- Assistance with everyday household routines
- Promoting choice, dignity, privacy and independence
The exact service, frequency and tasks depend on the person’s assessed needs, approved Support at Home care plan, available budget, Trilogy Care arrangement, location and CCTS service capacity.
